Undergraduate Program

• an application form
• a high school final certificate (original or duplicate issued by the school)/ a high school diploma
  - "new matura": original or duplicate copy issued by the school and the final school certificate (original or duplicate certified by the high school secretariat)
  - "old matura": original or duplicate copy issued by the school
  - in case of seniors graduating from foreign high schools: an original of the diploma with a certified Polish translation and a nostrification certificate issued by the Educational Authority (Kuratorium Oświaty)
• 2 coloured photographs
• a photocopy of national identity document or passport
• a photocopy of the registration fee receipt in the amount of 300 PLN
